This document, developed in Task 2 of HOIS project C21-05, presents specific new guidance to address variations in the quality and effectiveness of reporting of visual inspection or survey. The guidance has been developed from analysis of human factors issues in reporting, cited in discussions within the HOIS human factors working group, identifying improvements or mitigations that members have put in place using a failure modes and effects analysis (FMEA) approach.
The human factors guidance presented in this document is role-specific, applicable to process initiators and end-users, such as integrity engineers, inspection engineers in charge of inspection and personnel (inspectors) performing the visual inspection or survey.
