Failure Mode and Effects Analysis
FMEA – Failure Mode and Effects Analysis refers to a structured process for identifying how products, systems and processes may fail and establishing the consequences of such failures. It is generally conducted by a multi-disciplinary team that identifies the possible failure that could occur, the mode causing that failure, what will happen when the particular component fails and the consequential knock-on effect through the system. The probability of it happening and the severity of the consequences are assessed. Mitigations can then be implemented to avoid the failure or at least reduce both the probability of it happening and the
consequences.
